In-depth Synopse: (Spoilers for the Devil's Trill Arc)
Hijiri is a very talented violinist, the national champion in fact and the favourite of the principal at the prestigious music school he studies at. However, even though he's such an accomplished violinist, he is never arrogant and a very friendly and outgoing person who gets along very well with his classmates with the exception of a rich bully named Saionji who is extremely jealous of Hijiri. Hijiri is not afraid of him though and is one of the few if not only person, brave enough to stand up to Saionji.
Hijiri's life changes the day he gets a cornea transplant for the failing sight in his left eye. The cornea is cursed and belonged to another brilliant violinist, Tatsuya Otonashi, who made a deal with the demon, Surgatanus to exchange the thing most precious to him, his daughter Kazusa, for great violin skills. The deal was that upon Tatsuya's death, his daughter would be given to Surgatanus, however, though Tatsuya is dead, a part of him, his cornea, is still alive as it has been transplanted to Hijiri so the demon cannot claim Kazusa yet. In order to be able to get his part of the deal, the demon plans to kill Hijiri. At the same time, the Shinigami, Tsuzuki and Hisoka are sent to investigate Tatsuya's death with Tsuzuki posing a school nurse in order to keep an eye on Hijiri.
Hijiri wishes to thank the person who gave him his cornea, but since Tatsuya is dead, he visits the orphaned Kazusa instead (Hijiri is an orphan as well) and that night he feels very guilty. He feels as if he's killed Kazusa's father, but these feelings of guilt are actually caused by Surgatanus who is trying to drive him to suicide.
Hijiri goes back to school and is immediately told that he has been chosen to give the solo performance at the upcoming school concert. This makes Saionji even more jealous of Hijiri and he sets fire to the music room where Hijiri's violin is kept. His violin was a keepsake of his late father's so Hijiri falls into a depression. Kazusa offers him her father's violin (which is cursed) and Hijiri is immediately drawn to it, however, he becomes possessed whenever he plays it. Each time he's possessed, he will play 'The Devil's Trill', a beautiful yet very difficult violin piece composed by Giuseppe Tartini, which Tartini is said to have written after hearing the Devil play it in his dreams.
After being possessed during one music lesson in school, his classmates begin to fear him and distance themselves from him. Their behaviour adds to the guilt Hijiri already feels. Kazusa also seems very afraid of him all of a sudden (but that is only because she can see Surgatanus following Hijiri around in the form of a large, black dog). All this drives him to suicide.
He is saved though and taken to Enma-cho where Tsuzuki and Hisoka explain the whole situation to him. With Watari’s help, they find and decipher the terms of demon's contract with Tatsuya inscribed in Hijiri’s donated cornea. They are unsure of which demon made the deal though. However, Hijiri refuses to stay and hide in Enma-cho and insists on going back to the world of the living. Tsuzuki agrees to protect him from harm.
A series of bizarre freak accidents follow Hijiri after that, all Surgatanus' attempts to kill him. Tsuzuki gets slightly injured during one of his attempts to protect Hijiri, and this makes Hijiri realize that his stubbornness and refusal to stay in Enma-cho till they find the solution to his curse is causing those around him to get hurt. He finally agrees to stay in Enma-cho. Yet, that night just as he is practicing on a new violin (the cursed one has been taken away), Surgatanus visits him and gives the cursed instrument once again. Hijiri is once again possessed but luckily Tsuzuki stops him in time.
In the meantime, Kasuza, who has been taken to Enma-cho as well for her protection, recognizes the dog following Hijiri around as the demon, Surgatanus. Now that they know who they are up against, a trap is set for the demon. Hijiri plays the cursed violin to lure him out then Tsuzuki fights him. During the fight, Tsuzuki is bitten though the demon is defeated. He appears to be unharmed, but is already possessed by Surgatanus.
No one seems to notice that something is wrong with Tsuzuki except for Hisoka and he comes up with a plan to disguise himself as Hijiri. Tsuzuki, now possessed, kills Hijiri, who is actually Hisoka in disguise (Hisoka is alright since he has mad regeneration power, being a shinigami and all).
Tsuzuki/Surgatanus then goes to Enma-cho and demands to be given Kazusa now that he thinks Hijiri is dead. He then learns how Hisoka has tricked him and is not amused by that at all. He uses Tsuzuki’s shikigami, Suzaku and Byakko to attack Enma-cho. Hisoka attempts to free Tsuzuki from Surgatanus’control by performing a reibaku on him. Hijiri, seeing Tsuzuki in pain, stops Hisoka before he can complete the technique allowing Surgatanus to escape from it. The demon tells them that he is able to so easily take over Tsuzuki's body because he's trapped him in his memories of his past sins. Hisoka attempts a second reibaku and somehow (it's never really explained how...) Hijiri is able to enter the dream Tsuzuki is trapped in to reassure and comfort him. Tsuzuki is then able to break free from Surgatanus’ control and is attacked and killed by Suzaku and Byakko. During the fight, part of the ceiling collapses, falling right for Hijiri, Kazusa pushes him out of the way and gets killed instead.
Hijiri soon has to leave. Before leaving, he tells Tsuzuki not to feel guilty for what he did while he was possessed or for failing in his promise to protect Hijiri. He comforts Tsuzuki by telling him that he doesn't need to be protected anymore since he can protect himself. Then he hugs him and makes him promise to go to his concert.
The concert is a success as Hijiri is no longer cursed and is able to play 'The Devil's Trill' perfectly without any help from the demon. Tsuzuki keeps his promise and goes to the concert with Hisoka and Kazusa.
After the concert, Hijiri is taken to Luceti...
